16 employee flu shots remain for the 2017-2018 school year. The final walk-in clinic for flu shots will be held at the Student Health Center on Thursday, November 30th. First come first serve. Make sure to get your flu shot before they are out!

As part of the Affordable Care Act (ACA) legislation, employers are required to provide Form 1095 to employees who were offered and/or enrolled in employer health insurance. Employees who meet this criteria will receive their Form 1095 in March at the home mailing address on file at that time. Only one form is provided from UNI for all individuals listed on your policy; you may need to provide copies to your spouse or dependents, as necessary.

Human Resource Services is offering a training session on Leave requests under the Family and Medical Leave Act for UNI employees and supervisors to attend. The training session will be offered on Thursday, November 20 from 3:00 p.m. to 4:00 p.m. in the Center for Multicultural Education. This session will provide information on the University’s process, procedure, and policies related to the Family Medical Leave Act. Advance registration is required.
