How to Request a Public Service Loan Forgiveness Form

There are two ways to obtain a Public Service Loan Forgiveness Form.

  1. Access The Work Number Employee Portal through employees.theworknumber.com. In the “Find your employer” section, type in University of Northern Iowa and choose the correct option. Provide the necessary login information. If you are a First time User, you will need to register prior to logging in. Click on the Letters Center then on the PDF Icon next to Public Service Loan Forgiveness. Follow the instructions to generate your PSLF form.
  2. Use the PSLF Help Tool to submit your form electronically. You will need to log in with your account username and password (FSA ID). As you go through the tool, you will have an opportunity to apply your digital signature. Following that action, you will be presented with information and prompts to request your employer's signature. Please provide hris-support@uni.edu as the email address for the authorizing official to certify your employment.

Once we receive the electronic document we will verify your employment and digitally sign. Please note: if there is conflicting information on the form that we are unable to correct, we will decline signing and add a note to let you know what information needs to be changed. Once you receive the form back, correct the requested information and resubmit.

After we have completed the certification, the form will be submitted to the PSLF servicer via the PSLF Help Tool for processing.

If you have any questions about this process email hris-support@uni.edu.

Reminder: Do not email this form or other personally identifiable information (PII).
