Health Insurance Cards

First-time Users

Items needed to register: Wellmark ID

  1. Go to https://welcome.wellmark.com/Registration/Member/MemberRegistration.aspx
  2. Enter the information requested and click Next
  3. You will then be prompted to set up your username/email, password and security questions
  4. From the menu bar select the My Account button

  5. From the My Account options, select the Order ID Card button

  6. From the Order ID Card section, select the button View/print ID card
    View/print ID card button

Returning User

  1. Go to https://www.wellmark.com/ and click on Log In/Register at the top right of the screen
  2. From the menu bar select the My Account button

  3. From the My Account options, select the Order ID Card button

  4. From the Order ID Card section, select the button View/print ID card
    View/print ID card button

Dental Insurance Cards

First-time Users

Items needed to register: Delta Dental ID

  1. Go to https://login-wsprod.deltadental.com/subscriber_registration_controller.jsp
  2. Enter the information requested and click Register
  3. From the Quick Links section, select the ID Card link

Returning User

  1. Go to https://www.deltadentalia.com/ enter Username and Password and click on Log In
  2. From the Quick Links section, select the ID Card link

Vision Insurance Cards

First-time Users

Items needed to register: Avesis ID

  1. Go to https://www.avesis.com/Commercial3/Members/RegisterMember.aspx
  2. Enter the information requested and click the Submit & Get Started button
  3. Click the Print an ID Card button
    Print and ID Card button

Returning User

  1. Go to https://www.avesis.com/commercial3/members/index.aspx enter Username and Password and click on Log In button
  2. Click the Print an ID Card button
    Print and ID Card button
