How to Obtain New and Temporary Insurance Cards - A Helpful Guide

This page is designed to help you access or request insurance cards for the different university plans.

Navigate to a specific insurance plan:

Health Insurance Cards

New and Returning Users

Items needed to register: Wellmark ID

  1. Go to the MyWellmark login site.
  2. Enter the information requested and click Next.
  3. If a new user, you will then be prompted to set up your username/email, password and security questions.
  4. Once logged in, from home screen, click on ID cards.

  5. You may then choose to email a copy to yourself, or order a new card.

  6. If you are utilizing the Mobile App for MyWellmark, iPhone users have the option to add their card to their Apple Wallet.

Dental Insurance Cards

New and Returning Users

Items needed to register: Delta Dental ID #

  1. Go to the Delta Dental website
  2. Enter the information requested and click Register or Log In
  3. From the Quick Links section, select the ID Card link

Vision Insurance Cards

First-time Users

Items needed to register: Avesis ID #

  1. Go to the Avesis Website
  2. Click Create a New Login Account
  3. Click on Create Member Account
  4. Enter the information requested and click the Next button
  5. Set up security questions and preferences.
  6. You will receive an an email HPSmailSvc@VeriBen.Net to validate your account and set up your password.
  7. Return to login screen, and log in.
  8. Once logged in, select the Print an ID Card link at the top of the screen.
     

Returning User

  1. Go to the Avesis Website,  enter Username and Password for Returning User Login.
  2. Click on the Login button.
  3. Select the Print an ID Card link at the top of the screen.
