A Qualified Life Event

Note:This page provides information on when the change of address is a permanent move. For UNI Blue Advantage (HMO) Plan situations where a covered member will be out of the coverage area for at least 90 days, but will be returning, please apply for a guest membership.

My Timeline

4 Months Before Life Event

Create a checklist of items that might need changed when you move, the following items could be considered:

  • Will you need to consider a new plan, or newly enroll into UNI health, dental and/or vision plans? To see what plan would work best for you and your family please visit our website.
  • If you choose to enroll/change in a UNI plan, did you consider signing up for the flexible spending plans. The Health Care Flex Spending Account (FSA) will use pre-tax dollars to pay for co-pays, coinsurance, and deductible charges from your health, dental, and vision plans. The Dependent Care FSA will use pre-tax dollars to pay for daycare costs.
  • Review optional Voluntary Term Life Insurance coverage. You may increase, decrease, or remove the coverage due to the life event.
  • Review and update emergency contact information if needed. It is important to have accurate, up-to-date emergency contact information so contacts can be notified.
  • Update any tax forms for the new state.
  • Review and update home address information to ensure mailings are received at the new address.

3 Months Before Life Event

Review all insurance plans that you might be eligible for to determine what plan works best for you and your spouse/partner and possible dependents.

Health, Dental, & Vision Covearge

Are you eligible to enroll in/change any of the UNI health, dental, and/or vision plans? To see what plan would work best for you, your spouse/partner and possible dependents visit our website. Your health and dental premiums will be deducted using pre-tax dollars, which means premiums will be deducted from your salary prior to taxes being deducted. You may opt-out of this option by submitting the After-Tax Premium Option form. You can make changes to any of the above mentioned plans by logging into Benefits Self-Service. Your changes will be effective first of the month after the life event.

Spouse/Partner Coverage Comparison

Is your spouse/partner employed? Do they qualify for health, dental and/or vision coverage through their employer? If so, you will want to compare those options to the UNI plans to see what plan works best for you and your family. Coverage area and networks of each of the plans may help you decide based on your recent move.

Flex Spending – Health Care and Dependent Care

If you choose to enroll in, or change your UNI plan, did you consider signing up for, or adjusting, the flexible spending plans? The Health Care Flexible will use pre-tax dollars to pay for co-pays, coinsurance, and deductible charges from your health, dental, and vision plans. The Dependent Care FSA will use pre-tax dollars to pay for dependent daycare expenses. If your new health plan has different co-pay, coinsurance, and deductible amounts, you may want to consider changing your health care amounts. If you have to change the care for a dependent, and the cost is different, you may need to change the amount of your dependent care FSA. You can make changes by logging into Benefits Self-Service. Your deductions will start the first of the month after the election is made.

Voluntary Term Life Insurance and Accidental Death & Dismemberment

Review optional Voluntary Term Life Insurance coverage. You may increase, decrease, or remove the coverage for yourself, your spouse, or children due to a qualified life event. You can make changes by logging into Benefits Self-Service. Your change will be effective the first of the month after the election is made.

Beneficiaries

Review your current beneficiaries. Do they need to be updated?

You can update your term life beneficiaries anytime through Benefits Self-Service.

For IPERS beneficiaries, please fill out the IPERS Enrollment and Beneficiary Designation Form and submit to IPERS at the address on the form.

To update your TIAA beneficiaries please log onto the TIAA website and update your beneficiaries accordingly.

1 Month Before Life Event

When you have decided which plans you would like to enroll in, or make changes to, plan ahead and find out what the enrollment process is.

Documentation Requirements

If you decide to newly enroll into a plan through UNI you will need to provide the following documentation:

  • Lease or Rental Agreement
  • Mortgage Deed
  • U.S.Postal Service Change of Address Confirmation

1 Week After Life Event

Now that you’ve officially moved you should start making changes, if needed, to the topics that were covered above. Once you have the necessary documentation you will be ready to make your benefit and/or name changes via Benefits Self-Service. Please remember that you have 30 days (from the time of the life event) to make these changes.

Additional Personal Information Updates

Have any personal information that needs to be updated such as name, address, and emergency contacts? These can be updated by logging into My UNIverse and selecting Update My Personal Information.
